Real-world hepatic safety of the potassium-competitive acid blocker: A RUCAM-adjudicated multicenter cohort study.

New evidencehepatocellular carcinomaepidemiologybasic science
Clinical takeawayReassuring but inconclusive: RUCAM-adjudicated DILI occurred in 3, 2 and 3 patients per matched group (0.17, 0.07 and 0.15 per 100 person-years), so confidence intervals were wide (HR vs PPI 1.63, 95% CI 0.28-9.56). Choose acid suppression on indication and availability; this study removes a signal, it does not establish equivalence.
What it foundRUCAM-possible DILI occurred at 0.17, 0.07, and 0.15 per 100 person-years in tegoprazan, PPI, and H2RA groups, respectively, with no significant difference (tegoprazan vs H2RAs HR 0.98; 95% CI, 0.20-4.95; vs PPIs HR 1.63; 95% CI, 0.28-9.56).
ContextThis study addresses concerns from early-generation P-CABs' hepatotoxicity, providing real-world evidence that newer P-CABs like tegoprazan do not increase DILI risk compared to PPIs or H2RAs, though the rarity of events limits definitive conclusions.

Every paper is compared to the standard governing its question. This one is not: either no standard in the corpus matches it, or the comparison did not hold up on review and was withdrawn rather than published unverified. Both are logged.
